EFFECT OF L-CARNITINE ON THE INDICES OF ENDOTHELIAL DYSFUNCTION
AND INFLAMMATION FACTORS IN PATIENTS WITH ACUTE MYOCARDIAL
INFARCTION WITHOUT THE Q WAVE

Aim: To evaluate the influence of L-carnitine on inflammatory markers and indicators of endothelial function in patients with acute myocardial infarction without the Q wave.

Materials and Methods: 40 patients with verified diagnosis of AMI without Q-wave. All the patients were matched by sex, age, weight and duration of the underlying disease. The patients were randomized into 2 groups. The first group of patients were on a standard basic therapy and additionally administered L- carnitine at a dose 5g/day, II- group consisted of patients who did not receive drugs with metabolic action. In dynamics (on the 1st and 10th day of the treatment) the parameters of endothelium vasodilation and markers of inflammation were evaluated.

Results: On the background of the basic therapy in both groups an improvement in endothelial function resulting in an increase of brachial artery diameter was observed. However, the L- carnitine vasodilator effect was more pronounced and persistent as compared to group II. In the dynamics, CRP levels and plasma fibrinogen were more pronounced in the Ist group.

CLINICAL EFFICIENCY OF BISOPROLOL IN PATIENTS WITH CORONARY HEART
DISEASE DEPENDING ON POLYMORPHOUS MARKER TYPE OF ADRB-1 GENE

Abstract. At present, in order to influence the indices of the global contractions of the left ventricles in patients with coronary heart disease effectively, genetic characteristics in receptor structure and particularly adrenoreceptors responsible for sympathoadrenal activity should be considered. Patients division depending on the type of polymorphism gives prerequisites for new efficient algorithms for individual selection of medicines, including β-blockers. Taking into consideration the type of polymorphous marker of ADRB- 1 receptor, it is possible to maximize the effect of β-adrenoceptor, thereby to influence effectively the pathogenic mechanism of the disease.

Indices of bronchial lability in children with atopic and nonatopic
bronchial asthma depending on acetylation status

Abstract. After comprehensive clinical, laboratory and instrumental investigations of 51 children, who suffer from bronchial asthma, it was determined that in patients with atopic asthma slow acetylation status increased the possibility to detect a severe bronchial lability (bronchial lability index more than 30 %) by 4,4 times compared to the patients with fast acetylation status. For atopic phenotype of disease availability of correlations between bronchial lability indices and bronchial obstruction severity was established (r=0,7, p<0,005) as well as duration of illness (r=0,8, p<0,001), frequency of re-hospitalization of patients (r=0,6, p<0,05). In patients with nonatopic form of the disease relationships between bronchial lability indices and clinical features of disease depending on acetylation status haven’t been reported.

SEMEN ANALYSES EVALUATION IN MEDICAL CENTRE FOR INFERTILITY TREATMENT

Abstract. Semen analyses, made in Chernivtsi Medical Centre for Infertility Treatment from 2010 to 2012 were studied. The parameters were evaluated as recommended by WHO, 2000. 1,935 men where involved in the study. A general tendency of increasing asthenozoospermia in B category was found.

CORRECTION OF HEPATIC ENCEPHALOPATHY IN PATIENTS WITH LIVER
CIRRHOSIS IN COMBINATION WITH INTESTINAL DYSBIOSIS

Abstract. 65 patients with liver cirrhosis combined with intestinal dysbiosis have been examined; an assessment of the functional state of the liver, clinical manifestations of hepatic encephalopathy, neuropsychometric tests, intestinal microbiota has been carried out; Lactobacillus and Bifidobacterium as well as Methylsiliconic acid hydrogel were included into the complex treatment. Inclusion of these drugs in the complex treatment resulted in significantly better reduction of subjective and objective signs of cirrhosis, severity of functional and biochemical syndromes, symptoms of hepatic encephalopathy.

DIAGNOSIS OF GENITAL TRACT INFECTIONS IN PREGNANT WOMEN

Abstract. The article presents data on the diagnosis of genital tract infections in patients with pulmonary tuberculosis, whose pregnancy ended in premature delivery. It is shown that in this contingent of pregnant women, vaginal microflora undergoes qualitative and quantitative changes, early detection of which contributes to the effective prevention of a preterm birth.

CLINICAL EFFECTIVENESS OF COMPLEX TREATMENT OF PERIODONTAL
DISEASES IN PATIENTS WITH SCHIZOPHRENIA

Abstract. Treatment schemes and medical prophylaxis of inflammatory-dystrophic periodontal diseases in patients with schizophrenia are pathogeneticyally considered and developed. The efficiency of the suggested method of complex treatment of periodontal diseases has been proved by the reduction of therapy terms, prolongation of clinical stabilization period, as well as normalization of clinical and paraclinical indicators in patients treated in all terms of supervision.

The sex related effects of intermittent hypobaric hypoxia on the
oxidized modification of proteins in the gingival tissues in immature
rats under conditions of changed photoperiod

Abstract. It is shown in experiments that intermittent hypoxia (equivalent to the altitude 4000 meters, 2 hours per day for 14 days) changes the capacity of the oxidized modification of proteins (OMP) in the gingival tissues of immature albino rats. The result of hypoxic effects depends on the sex of animals: in males it causes a decrease of proteins peroxidation intensity both of basic and neutral nature; in females – only a tendency to decrease of the basic OMP.

Functional state of the pineal gland changed by experimental conditions (14 days of illumination – “physiological pinealectomy”, or 14 days of darkness – stimulation of the pineal gland) influenced the effects of hypoxia on the OMP in the gingival tissues. The darkness heightens the decreasing effects of hypoxia on the capacity of OMP in the gingival, which is more pronounced in male rats.

Our results suggest that by an application of combination of intermittent hypobaric hypoxia and the darkness it is possible to prevent the process of accumulation of OMP in the gingival tissues

ARTERIAL HYPERTENSION IN WOMEN IN THE CLIMACTERIC PERIOD: CLINICAL,

MATHEMATICAL COMPUTER-BASED MODELLING OF BIOMECHANICAL
BEHAVIOR OF INTRAMEDULLARY NAILING FOR SUPRACONDYLAR
FEMORAL BONE FRACTURES

Abstract. The research of the mode of deformation of the “femur – fixation device” system is done for intramedullary nailing of the distal end fractures of the femur (fracture A1 in AO classification) by the finite element method. The models differ by their distance between the plane of fracture and the articular surface of the femur: model 1 – the plane of the fracture is 5 cm from the articular surface of the femur; model 2 – the plane of the fracture is 6,5 cm from it; model 3 – the plane of the fracture is 8 cm from the articular surface of the femur. The received results convincingly demonstrate the advantage, both in terms of strength and stiffness, of the connection formed by intramedullary nailing on the distance of 8cm (model 3).

INFLUENCE OF LONG LIPID LOWERING THERAPY ON THE MICROELEMENT

Hyperglycemia as a component of metabolic syndrome and
the main important factor of its progression

Abstract. The results of the investigation show that the primary disturbances of the carbohydrate metabolism,
namely – a moderate hyperglycemia as a result of chronic glucose overload led to the complex of proatherogenic disturbances which are typical for an insulin resistance. It was also established the close connection between the syndrome components which is realized through such main mechanisms as hypertriglyceridemia
(HTE) systemic inflammation and oxidative stress. These data show the independent significance of hyperglycemia as a factor which initiates the progression of metabolic syndrome.

STATE OF THE BLOOD LIPID PROFILE IN ACUTE PERIOD OF RECURRENT
ISCHEMIC STROKE

Abstract. In patients with both primary and recurrent ischemic stroke in its acute period the combined dyslipidemia with no probable differences was found which declared itself in an increase of general cholesterol, cholesterol of low density lipoproteins, cholesterol of extra low density lipoproteins, triglycerides and atherogenic factor at low concentrations of cholesterol of high density lipoproteins.

There were no significant differences found between the indexes of lipidogram of different levels of severity in the primary ischemic stroke group as well as in the group of recurrent ischemic stroke, and no significant differences between their same name levels of severity were found either.

EFFECT OF METHANOL ON THE ULTRASTRUCTURE OF CHORIORETINAL
COMPLEX OF RATS’ EYES IN THE EARLY STAGES OF THE EXPERIMENT

Abstract. Electron-microscopic structure of the chorioretinal complex were investigated (СRC), [choriocapillaries (HC) – retinal pigment epithelium (RPE) – photoreceptor cells (FC)], of white rats in a period of 40 min. to 3 days after a single intraperitoneal dose of methanol 0.75 g/kg body weight. It has been established that RPE cells are the most responsive to the dose of the methanol used. In the dynamics (from 40 min. up to 3 days), they grew destructive changes of mitochondria and elements of smooth endoplasmic reticulum, the smoothness of the basal folds and patchy destruction of the apical microvilli. The changes in CC and FC were similar. By the end of the observation period, these phenomena in the CRC structure spread to a larger number of cells. At the same time, during the whole period of the study, and, in particular, after a day, some signs of recovery of compensatory nature were obvious. Attention is drawn to pronounced reaction of mitochondria, which are energy forming structures of a cell.

DIAGNOSING ARRHYTHMIAS WHILE SCREENING SCHOOLCHILDREN

Abstract. The study involved 420 children of school age. The children underwent screening determination of arterial pressure and heart rate using automatic blood pressure manometer with arrhythmias sensors. The results were specified by means of recording ECG on a portable cardiograph. The most common heart rhythm disturbances in schoolchildren were respiratory arrhythmia and bundle branch block.

DIFFERENTIATED COMBINED THERAPY OF POLYCYSTIC OVARY SYNDROME
FOR PATIENTS WITH OBESITY

Abstract. Obesity is one of pathognomonic symptoms of polycystic ovary syndrome (PCOS). According to different data sources its frequency among patients with PCOS ranges from 45 to 50 %. Efficiency of differentiated complex treatment of patients with PCOS is studied with taking body mass index (BMI) into account. Obtained data show positive effect of combined infertility treatment that included Metformin for insulin resistance correction as menstrual function and ovulation normalization in 58,7 % of patients with PCOS. Treatment with Metformin results in normalization of luteinizing hormone level which has explicit correlation with risk of 1st trimester miscarriages.

Abstract. An investigation of interconnection between anamnestic and clinical peculiarities of premenstrual syndrome (PMC) in edematous form and hormonal homeostasis indexes in 130 women of early reproductive age has been carried out.

The correlation between anamnestic data connected with mental and emotional strain, symptoms which accompany the tissues edema and progesterone, prolactin and cortisol indexes in women with PMC edematous form is traced.

Abstract. 44 patients with essential hypertension have been examined in whose blood the indices of lipid peroxidation, аntioxidant system, level of basal insulin have been studied. It has been established that an activation of lipid peroxidation and decrease of аntioxidant system occur in patients with essential hypertension. Basal hyperinsulinemia in patients with essential hypertension is associated with an aggravation of these disorders.

The optimal choice of antihypertensive therapy in patients with
comorbid diseases: chronic obstructive pulmonary disease and
essential hypertension. Justification is based on personal experience

Abstract. The aim of our study was to evaluate the effectiveness of various schemes of antihypertensive therapy in patients with chronic obstructive pulmonary disease and concomitant hypertension. Three different schemes of combined therapy with antihypertensive drugs have been assessed. It has been established that prolonged use of a combination of ACE inhibitors and beta –blockers caused a significant decrease in heart rate, in the number of episodes of painful and silent myocardial ischemia, ventricular extrasystoles of high grade. When combined, ACE inhibitor and diuretic decrease the value of the functional class of heart failure, the positive inotropic effect and the quality of life parameters improve significantly. A combination of ramipril and amlodipine had much better antyremodulating effect, and the pressure in the pulmonary arteries dropped. At the same time, all three therapies equally effectively reduced the level of office pressure.

SMALL SAPHENOUS VEIN AND ACUTE VARICOSE THROMBOPHLEBITIS
IN LOWER LIMBS

Abstract. In order to compare the role of the small and great saphenous veins (SSV and GSV) in the development of acute varicose thrombophlebitis in the lower limbs (AVTLL), 236 patients with thrombotic process in 250 limbs have been examined. The number of frequency of thrombosis among the limbs with reflux both in SSV and in GSV presented the role of these veins at AVTLL most objectively. According to this value, there was no statistically significant difference between SSV and GSV (p=0,669): 49,0 % (95 % CI 34,8-63,4 %) versus 45,1 % (95 % CI 31,1-59,7 %). This result showed that traditionally declared higher frequency of thrombosis in GSV among the total number of patients with AVTLL was caused by the more frequent presence of abnormal reflux in this vein solely. The frequency of thrombosis in the tributaries of GSV was significantly higher than the frequency of thrombosis in SSV tributaries, regardless of groups which were compared (all p<0.05). Results of the study indicate that the role of SSV at AVTLL is often underestimated, and a thorough examination of the pathology of its system can improve the treatment effects of the patients with AVTLL.

THE EFFICACY OF THE SYMPTOMATIC THERAPY OF THE NON-ATOPIC
BRONCHIAL ASTHMA ATTACK IN SCHOOLCHILDREN

Abstract. The effectiveness of symptomatic treatment of bronchial asthma in children has been studied. It has been found that patients with non-atopic asthma phenotype were characterized by more severe bronchial obstruction during exacerbations compared with atopic patients. Children afflicted with nonatopic phenotype of the disease were characterized by more pronounced signs of airway obstruction during the whole hospitalization period despite the identical standard regimens in the comparison groups.

USING CELL INDIRECT REVASCULARIZATION FOR CHRONIC LIMB ISHEMIA

Abstract. The problem of treating patients with no reconstruction of peripheral arterial bed is relevant and it still remans open. The results of the pilot study with a simulated limb ischemia, demonstrated the feasibility and efficacy of fetal liver progenitor cells in activating regenerative and recovery processes as well as  angiogenesis.

The aim of the research was to investigate the clinical efficacy of cell "indirect" revascularization method in the complrehensive treatment of patients with chronic limb ischemia.

The developed method of fetal human liver progenitor cells application in treatment of lower limb ischemia is an effective and affordable method of indirect revascularization. This method is indicated for use in patients whose vascular bed can not be reconstructed, and in elderly patients with severe concomitant pathologies.

Abstract. Levels of GDF-15, NTproBNP, structural and functional changes of the heart were assessed in 69 patients with postinfarction cardiosclerosis and preserved left ventricular ejection fraction (EF>45 %). We found out that patients with heart failure and preserved ejection fraction after myocardial infarction on the background of arterial hypertension have increased levels of GDF 15 and NTproBNP. These biomarkers were correlated with left ventricular diastolic dysfunction and left atrial volume index.

correction of hormonal homeostasis by oral contraceptives
after medicamentous termination of pregnancy

Abstract. We evaluated the effectiveness of an application of middle-dose monophasic oral contraceptives (aethinyloestradiolum, desogestrel) for the maintenance of hormonal homeostasis and treatment of the most common complications that occur in the first three months after abortion. The use of oral contraceptives on 7-9th day caused the termination of blood discharge, signs of mastopathia and normalization of menstrual cycle. Thus, administration of drugs maintaining hormonal homeostasis is recommended as an effective method of correction of complications of pregnancy termination in early terms.

Abstract. This article presents and analyzes the data from a study of the immune status and the role of immune factors in the progression of polyneuropathy in patients with type 2 diabetes mellitus. The relationship between markers of the functional state of myelin and some parameters of the immune system are ascertained. The most sensitive are tibial nerve characteristics and in most cases – the residual latency period, that is the functional state index of myelin of the most dystal nerves’ segments. It was found that the progression of the polyneuropathy severity is accompanied by the secondary demyelination as a result both of the deepening axonopathy process and some immune disorders.

Optimization of therapeutic tactics in patients with acute

Abstract. 202 patients with acute small bowel obstruction (ASBO) were involved in a study of clinical and biochemical parameters that characterize the state of endotoxicosis. It has been established that the dilated afferent segment of the small intestine is one of the sources where endointoxication is formed and its functional state, due to an imbalance in the morphological structure, has a direct impact on the surgical treatment results.

FASL-MEDIATED APOPTOSIS IN CHRONIC CARRAGEENAN-INDUCED
GASTROENTEROCOLITIS

Abstract. The levels of sFasL and caspase-3 activity were studied in blood serum of rats with chronic gastroenterocolitis developed after intake of carrageenan during 2 and 4 weeks. It was established that the disease was accompanied by increased activity of caspase-3, more considerable in case of more prolonged carrageenan consumption suggesting activation of apoptosis in chronic intestinal carrageenan-induced inflammation. The elevated level of sFASL was observed, which was higher after 2 weeks of carrageenan intake. It indicated the predominance of FasL-mediated apoptosis of enterocytes in the early stages and the significant role of other mechanisms of apoptosis in the later stages of chronic carrageenan-induced intestinal inflammation.

Changes in the platelet-vascular hemostasis influenced by basic treatment of 102 patients with acute myocardial infarction (AMI) depending on allelic variants of genes angiotensin-converting enzyme (ACE, I/D) and endothelial NO-synthase (eNOS, 894G> T) have been analyzed. The dependence of the treatment efficacy on genotypes and haplotypes of the analyzed genes in relation to hemostasis key-chains has been established.

OZONE THERAPY IN TREATMENT OF PATIENTS WITH UROLITHIASIS COMPLICATED CALCULARY PYELONEPHRITIS AFTER PERFORMING SHOCK-WAVE LITHOTRIPSY

Abstract. 35 patients were involved in a study of the effect of the use of intravenous ozone therapy within 10 days for a kidney stone disease with calculous pyelonephritis after applying the distance shock-wave lithotripsy. The positive therapeutic effects of its application , which was manifested in a decrease of lipid peroxidation products in the blood and increased activity of antioxidant enzymes has been established.

THE CHANGES IN FIBRINOLYTIC SYSTEM OF THE BLOOD IN THE PERIOPERATIVE

Abstract. The authors have investigated the dynamics of the parameters for the fibrinolytic activity of the blood serum in operative treatment of patients with benign prostate hyperplasia. Surgical interference was performed to the extent of transurethral resection of the prostate gland. The peculiarities of the state of fibrinolysis during the intraoperative and postoperative periods have been ascertained. The state of fibrinolytic activity of the blood serum in patients with benign prostate hyperplasia and the control group, comprising men of identical age, has been analyzed.

CORRECTIVE INFLUENCE OF ADEMOL ON METABOLISM OF
NITROGEN MONOXIDE IN THE BRAIN OF THE RATS WITH
MODELING CEREBRAL ISCHEMIA

Summary. In experiments on the rats with model of acute disorder of encephalic circulation (bilateral carotid occlusion) it was established that introduction of derivate of adamantan 1-adamantilethyloxy-3-morpholino-2 propanol hydrochloride (under conventional name ademol) in dose 2 mg/kg intraabdominal in treatment mode (in an hour after insult and further 1 time a day after every 24 hours during 21 days of cerebral ischemia) better than cerebroprotector citicoline provides decreasing of activity of general NO-synthase (NOS) in the rats’ brain, on which indicated reduced content of stable metabolite of NO and preservation of L-arginine (L-Arg) pool. Such effect of ademol can be one of the key mechanisms of its protective influence on ischemic neurons of the brain and indicate a perspective for creation on its base a new cerebroprotector agent.

The results of a study of morphometric parameters and structure of the spinal cord segments of human fetuses without abnormalities in the central nervous system compared to a human fetus with myelomeningocele and hydrocephalus are presented in the paper. Some changes both in morphometric parameters values, and in the internal structure of some spinal segments have been detected.

Time organization of physiological functions in mammals.
Cerebral structures INVOLVEMENT

Abstract. The results of five year research concerning the mechanism providing circadian rhythms of physiological functions of organs, in particular the kidneys, are presented in the paper. It was proved by light optical, electron microscopy, biochemical and immunohistochemical methods that structures of the organs under the study as well as their functions are strongly subordinate to the duration of the photoperiod. Depression of the pineal body activity (24 hour lighting) causes desynchronization in the structure of suprachiasmatic nucleus (SCN) neurons and in protein c-Fos synthesis. Disturbance of the diurnal rhythm of the SCN pacemaker and of melatonin synthesis is conditioned by the change in the rhythm of epiphysis secretory cells. Besides the SCN and the pineal gland, the hippocampus neurons are involved in the photoperiod disintegration.

OPTICAL SPECTROSCOPY IN MEDICAL DIAGNOSTICS

Abstract. In this review we consider the possibility of optical spectroscopy techniques, namely: optical tomography, optical coherent tomography, polarization-sensitive optical coherent tomography, Raman spectroscopy and photoacoustic tomography. The physical basis of optical diagnostic techniques is the interaction of light with matter, allowing of obtaining structural, biochemical, morphological and physiological information as a result of absorption, scattering, reflection of electromagnetic radiation in the optical range. The advantages of these methods, such as non-invasiveness, absence of side effects, contactlessness, high sensitivity, and high resolution are keys to their success in medical diagnostics.

Abstract. One of the important challenges that our society faces as a whole and medicine in particular is the formation and development of prenatal culture (or parenthood culture) which will help to raise a generation which treats the childbirth as natural and joyous event in their lives. Childbirth is an expected, mysterious and unforgettable stage of life for each woman. Pain will be soon forgotten – it’s the woman’s nature and the sole moment of the birth of a little wonder will remain in her memory. For the labour to be normal, we should pay special attention to psychological preparation of the pregnant woman. The article deals with the problems of the woman’s attitude to her pregnancy, to the labour and delivery. These data could be used in maternity and paternity schools while developing the curriculum.

Independent work is carried out by using methodical instructions for students which include educational aims, a list of questions, situational problems and tests for self-control as well as references. This kind of study stimulates students to the individual work with literature, to searching information on the Internet, making up abstracts and writing notes. A scientific students’ circle was created at the department in order to increase students’ independent work effectiveness and to strengthen their motivation to perform the work.

Abstract. Ukrainian society has entered the phase of reformation of higher education, including medical education, according to the standards of Bologna process. Our state faces presently the choice of new educational paradigm which aims at removing disparity between the requirements of the society and the level of educational services, at replacing the aim of comprehensive personality development with the aim of maximal development of person's abilities to self-realization and self-education. The importance of Bologna process is shown in the article; it is proven that it is an extraordinarily difficult and prolonged process, which requires, first of all, the correction of psychological and pedagogic culture of teaching staff of higher medical institutions, which is ready to reconstruct cardinally the educational process. It is difficult to over-estimate the urgency of the problem, as it concerns both the education and its reformation, as well as the process of integration of our country into Europe.

METHODICAL APPROACHES TO Teaching chemistry for the preparatory
department for ukrainian citizens in bukovinian state medical university

Abstract. Methodical approaches to teaching chemistry under conditions of limited amount of teaching hours for students with different learning abilities that are studying at the preparatory department for Ukrainian citizens of Bukovinian medical university are considered in this article. Based on the results of mental activity diagnosis for each group of teenagers methodical techniques for working with students were developed. It is proposed to use explanatory, illustrative training with problematic elements while teaching basic, principal topics, and for the sections that are a logical conclusion of basic topics to use the method of proactive knowledge for the groups of students with creative and constructive mindset. Systematization and structuring of studied material should be conducted through usage of graph-logic schemes and tables. It is advisable to practice skills in all groups of students regardless of their level of knowledge wherein for the groups with creative and constructive mindset tasks modeled from real situations and problems of increased complexity should be solved. Extracurricular independent work should be organized with different learning abilities of students taken in mind by developing tasks that are differentiated by their difficulty rate.
